When attempting to load a project containing more than two instances of Play_VST in Bridged Mode, the message "This plugin failed to load: (VST)" is displayed as seen in the attached jpeg image. This can be reproduced using track cloning by cloning the first bridged instance twice before the UI for the second instance is displayed.
