DirectWave, sliding to wrong pitch

So using DirectWave, I'm playing a looped bass sample.
I'm sliding from F-4 to a D#5. No big deal.

Except, when the slide finishes, the note plays at a C#5 pitch.

Pretty much, when I place the slide note it "preview-plays" the right pitch, but in the song it slides two semitones too low.

I'm assuming there's something about the settings I made in DW or the sample itself, even though I used a plain old .wav sample.
Edison tells me it's a 44.1kHz, 16bit file and when used in a new instance of DW, it slides just fine.

I've looked at the "note fine pitch" and it's all set to 0.
Legato time is .066 s so that's not it either.

Can anyone help me?

I've attached the (stripped down) .flp Play the pattern and hit the C#5 and D#5 note to see/hear it in action.
