two problem in sub of sub category in category page

Hi sir good evening
two problem in sub of sub category in category page

1 problem
when i create sub of sub category in sub category
then only show 1 sub of sub category
i have created 4 sub of sub category in sub category
it only show 1 sub of sub category

2nd problem
in sub category is empty all the content in sub of sub category
when i click on sub category then page refresh

and i have send your user name or pswd for more clarify

customer very disappointed for this can rectify it quickly
