Shipping rules for countries - any way to add more rules?

My client wants to charge shipping via country since she ships internationally. However, when I choose ship by country in the shipping settings, it lets me set a country and a price.

What about if users buy more than one product - how do I set a rule where 1-5 products = $10 and 6-10 products = $20, etc - but still choose countries because within the USA is quite inexpensive, (so they use USPS), but to ship to any other country is expensive and requires tracking (so, use FedEx).

However, shipping will cost more than $10 for a few products, so I need to set a quantity rule AND a country rule. How can I do this?
