unsibscribe default message

I lost the default message of the Unsubscribe notification in EZG while I was command+c the selection. Would you mind copy it and place it here so I can past it to EZG?

Also, When i get a unsubscribe notification email, I get the admin notification default message which is confusing for the admin as they might think this a subscription: "New visitor has just subscribed at %%Sitename%%: %FORM_DATA%"
What is the best to deal with it?
