Template from Version 3 Problems

I am using a template from Version 3 and have had no problems with it in Version 4 until today. No matter what I do the header, menu and footers will not upload. Errors can be seen at my website at http://www.survivorsofpolio.com

I am really worried, can someone please tell me what to do to correct the corrupt programming or whatever is causing the problem. Not a very good way to start my New Year.
