Multiple or single checkbox

I just finished with one customer request form and wondering it's possible to add multiple and single choice answers in request form.
something like --->> ONLY ONE SINGLE ANSWER CHECKING checkbox or radio field.

I know it is already there, but it's not possible to add only one single checkbox required. EZG do only either they must required all or not.
When someone check one or more checkbox need to be required. I am talking about just like survey forms.

AS YOU SEE ALL THE CHECKBOX FORM BELOW ARE NOT REQUIRED. I JUST WANT ALL MUST BE REQUIRED.
