Problem with Forms

Hi,

I have some sites which contain requestforms.
These forms have been created with the form widget, as it used to be!

Now I am doing maintenance on this site, and will have to add a form...
But there is no way I can use my same (old) layout again!. Suddenly the widget has been changed, and doesn´t give me the opportunity to create the "old-type" forms.
And this is something I want in the maintenance of the site. I don´t want to replace all other forms for consistency of look and feel.
Therefore are the layout options of the new forms quite restricted, and don´t always match the look and feel of the site.

Maybe, I am overseeing some options?
