the steps to duplicate this bug are as follows
create a single row, 2 column table, (number of rows doesn't really matter)
now select the full table and go into table properties, go to effects / rollover and enable it,
for type, choose column or cell,
(you might want to add block so you can easy see the table, but not part of the bug)
now click ok, go to browse mode, it should detect both columns, now head back into edit mode, right click on either cell / column, and insert a new column, (doesn't matter which cell you use) now head back into browse mode, when you hover you mouse over the table, it still thinking there are only two columns, (when there are in fact 3) inserting rows are fine, its just inserting columns, which aren't detected by the rollover, (it does detect the currect number of columns, if the starting table has a higher number, but inserting more, doesn't work
