If an audio clip is added using "insert->audio clip" and the sample is loaded in the "sample settings" tab of that audio clip, the entry in the "picker: audio clips" section of the playlist just displays "Audio Clip" instead of the sample name.
When a second audio clip is added, the first audio clip now shows the proper sample name but has no audio clip icon on the right hand side.
The problem does not happen when directly dragging and dropping a sample from the browser to the playlist.
Another remark is that when clips are added using "insert->audio clip", they are always gray, whereas when you add them by dragging/dropping they always have an automatically generated colour.
