Drumaxx: notes disappearing, crashes

I'm using Drumaxx (1.3.5) on a Mac (10.10.5) in Ableton Live (9.6.7). I think I'm mostly using the AU plugin, but may also have some presets with the VST version.

Recently, it has started:
* Loosing the Midi Note setting for the first 'pad', and even when this is set to the correct value, the pad does not trigger
* Crashing Live on unloading (or loading)

I'm using it in Instrument Racks, with a few plugins afterwards. Some of these are M4L, but I have had dysfunctional Racks with no M4L plugins present.

Additionally, quite often the plugin switches to only coming out of either the left or right channel, but this has been happening ever since I got it - all my Racks have an emergency mono switch on them to deal with it.
